What does it mean?
If you are wondering what ".doc" ".pdf" and "fillable.pdf" mean, this will hopefully help you out with the questions.

.doc - This is a form designed in Microsoft Word. This form should be easily accessible to most users although the appearance is not as "pretty" as a pdf document and the format may change slightly when you open it. You probably won't have any luck typing information into the fields without causing changes to the format of the form. This form generally should only be printed off and filled in by hand.

.pdf - This is a form that you can open with Adobe Acrobat Reader. These forms generally have a nicer appearance and the format remains consistent when you open the form. This version of pdf forms do not give the option of typing any information into the form and can only be printed and hand filled.

fillable.pdf - This is the ultimate of the forms! These forms can be opened with Adobe Acrobat Reader and you can type in the information in the fields and then print and mail the form to us. As you may guess, we much prefer this version to the others so that we don't have to decipher everyone's wonderful handwriting. Some of you may also have a little checkbox along the top when you open the form that when checked will highlight the fields that need to be filled in. The drawback is that it may take longer to download this form (but well worth the wait).
